Academic Dismissal Petition for Reinstatement

Note: Your hold will be removed after December 1st if you were dismissed for the current semester. You must submit a petition for reinstatement form by email or in person to have your hold removed.
Purpose: Petition for reinstatement following Academic Dismissal to remove enrollment block
Location: Online or Evaluations Office, SSC-40

Documentation Required: None
Processing Timeframe: December 1st for current year dismissals, or within 1-2 business days for any previous year dismissal
Student Notification: Email
Academic Review Committee Petition
Purpose: Request an exception to an academic policy or procedure
Location: Online or Evaluations Office, SSC-40
Location to Submit: Evaluations Office, SSC-40
Documentation Required: Varies with each student’s reason for petitioning
Processing Timeframe: Typically 1-2 meetings per semester, usually near the end of the term
Student Notification: Mail
Course Repetition Petition

Valid conditions for requesting course repetition
Purpose: Petition to demonstrate the necessity to retake a course in excess of the allowable limit or retake a course you have already passed
Location: Online or Evaluations Office, SSC-40

Documentation Required: Documents to support extenuating circumstances; see Section 5 of the College Catalog for a description of valid circumstances
Processing Timeframe: Typically 1-5 business days

Course Substitution/Waiver PetitionPurpose: Obtain permission from an academic department to substitute or waive a course required to complete an Associate Degree or Certificate of Achievement program
Location: Online or Evaluations Office, SSC-40
Location to Submit: Evaluations Office, SSC-40
Documentation Required: Unofficial transcripts and/or course descriptions
Processing Timeframe: Depends on academic department, typically up to 2 weeks
Student Notification: Typically student initiated or through degree or certificate program evaluation
Duplicate Diploma Request

Purpose: Request a copy of your diploma or certificate award
Location: Online or Evaluations Office, SSC-40
Location to Submit: Evaluations Office, SSC-40
Documentation Required: Possible name change documentation
Processing Timeframe: Typically within 1-3 weeks
Student Notification: Mail, or phone call to pick-up
Enrollment Priority Petition (Evaluations Office)
For Group 2 Priority based on student’s last term of enrollment prior to graduation or transfer

Note: This is a once-in-a-lifetime request
Purpose: Declare a registration term as your last term of attendance prior to graduation or transfer in order to receive priority enrollment for that term. This is a once-in-a-lifetime petition for this type of priority enrollment.

Documentation Required: Copy of current Student Education Plan
Processing Timeframe: Typically 1-2 business days

International Evaluation Agencies Purpose: Request an evaluation for coursework completed in a foreign country.
Graduation Petition

• Associate Degree (60+ unit program)
• Associate Degree for Transfer (60 unit program)
• Certificate of Achievement (18+ unit program)
• Certificate of Proficiency (less than 18 units)
• Transfer Certification (37-39 units)
Purpose: Apply for degree or certificate of achievement award, or for general education lower-division certification for transfer to a university
Location: Online or Evaluations Office, SSC-40

Documentation Required: Possible official transcripts, test scores
Processing Timeframe: Approximate timeframes for each term:
• Summer: 6-8 weeks
• Fall: 6-12 weeks
• Spring: 6-16 weeks

Requisite Clearance Request


Note: The Evaluations Office clears prerequisites for the following courses only: ACCT 202, ANTH 100L, BIOL 118L, 200, 201, 211, 212, BUS 205, CHEM 115/CHEM 115L, CHDV 105, GEOG 100L, OCN 100L, PSYC 210, PSYC 225 and PSYC 230 only.

Purpose: Request a course evaluation to meet prerequisite coursework for specific courses.

Processing Timeframe: 1-2 business days
Student Notification: Email
Requisite Challenge PetitionPurpose: A student has not completed the requisite at Palomar or at another institution but believes he/she has sufficient reasons to be allowed to enroll in and complete the course. A student may also challenge the requisite based upon ableism, availability of requisite, District processes, health and safety, and Title V violations. See form for details regarding acceptable challenge requirements.
Location: Online or Evaluations Office, SSC-40
Location to Submit: Submit to Academic Department Chair for review. Final, signed petition must be submitted to the Admissions Office with an accompanying Add slip to register in course.
Student Notification: Student will be notified of decision once verified by both the Department Chair and Division Dean.
