Academic Dismissal Petition Purpose: Petition for reinstatement following Academic Dismissal to remove enrollment block

Processing Timeframe: December 1st for current year dismissals, or within 1-2 business days for any previous year dismissal

Academic Renewal Petition Purpose: Petition to exclude prior substandard course work from a student’s grade point average which is not reflective of their current demonstrated ability.

Academic Review Committee Petition Purpose: Request an exception to an academic policy or procedure

Course Repetition Petition Purpose: Petition to demonstrate the necessity to retake a course in excess of the allowable limit or to retake a course that you have already passed

Course Substitution/Waiver Form Purpose: Obtain permission from the academic department to substitute or waive a course required to complete an Associate Degree or Certificate of Achievement Program

Enrollment Priority Petition

For Group 2 Priority based on student’s last term of enrollment prior to graduation or transferNote: This is a once-in-a-lifetime request
Purpose: Declare a registration term as your last term of attendance prior to graduation or transfer in order to receive a priority enrollment for that term. This is a once-in-a-lifetime petition for this type of priority enrollment

Purpose: Petition for an exception from loss of priority due to high unit majors or extenuating circumstances

Financial Aid Appeal Purpose: Petition if you have been denied financial aid because you have not made Satisfactory Academic Progress or you have earned an excess of the allowable units. This is the first level appeal.

Grade Adjustment Form Purpose: Request an adjustment to your grade point average after repeating a course in which you have received a substandard grade (D, F, FW)

Grade Dispute Form Purpose: Submit this form for a formal hearing to dispute a grade if you are not satisfied after you have discussed your grade with your faculty member, department chair and Dean.

Certificate of Proficiency Petition

  • Certificate of Proficiency (Less than 18 unit program)
Purpose: Apply for a certificate of proficiency award

Health Fee Exemption Petition Purpose: Submit this form if you are the member of a church that depends exclusively on prayer for healing in accordance with the teachings of a bona fide religious sect, denomination or organization.

Incomplete Form Purpose: Request to receive an Incomplete grade in a course due to unforeseen emergency, and justifiable reasons at the end of the term. Student will have 1 year to complete the missing course requirements. Failure to complete the course work will result in an F grade.

K-12 Special Admission Approval Form Purpose: This form is required for students who are under 18 years of age on the term or summer session start day.

  • K-12 students are limited to 7 units in spring or fall term and 5 units in summer session. A student requesting enrollment in excess of these limits must petition to the Director of Enrollment Services and provide transcripts and written justification. See K12 Special Admission Unit Petition
  • If the minor student is attending high school, the parent or guardian and the high school district official must sign.
  • If the minor student has graduated high school or has received a California High School Proficiency Exam Certificate, then only the parent or guardian signature is required.
  • If the minor student has been home schooled, then an official from the high school district where the student resides must sign and the parent or guardian must sign. (Private School Affidavit is Required)
  • If the minor student is under 16 years of age, then permission to attend must be requested of the Palomar faculty member who will sign the form if permission is granted. Admissions staff will process the enrollment in person.
  • In all cases, submit the form to the Admissions/Financial Aid Office in the Student Services Center in San Marcos or the Escondido Center.

K12 Special Admission Unit Petition Purpose: Effective spring 2014, K-12 students are limited to 7 units in spring or fall term and 5 units in summer session. A student requesting enrollment in excess of these limits must petition to the Director of Enrollment Services

Loss of BOG Fee Waiver Appeal Purpose: Effective Fall 2016, students that have been on two consecutive semesters of academic probation, progress probation, or a combination of the two, will lose their BOG fee waiver eligibility.  Submit this form and supporting documentation to appeal the loss of BOG fee waiver based on an extenuating circumstance or significant academic improvement.

Parking Citation Dismissal Petition Purpose: Submit this form to request dismissal of a parking citation.

Petition for Withdrawal Purpose: Submit this form to request an exception to withdraw from a class after the 50% point of a class.  The student may submit a petition under these circumstances:

  • Illness, hospitalization, or medical reasons
  • Employment
  • Relocation
  • Illness or death or a family member or loved one
  • Incarceration

If approved by the faculty member, the grade will be changed to W on the student transcript.

Processing Timeframe: Records will send the petition to the faculty member within 1-2 business days. Petitions are typically returned within 2 weeks. Prerequisite/Corequisite Challenge Petition Purpose: Complete this form to challenge the prerequisite or corequisite of a class.

Petition for Refund or Waiver Purpose: Complete this form when you are unable to drop classes during the first 14 days of the term or session due to a documented unforeseen emergency or extenuating circumstance. Drops made within the first 14 days of the term do not require a petition form. Students are automatically refunded by the Cashiers Office after the refund deadline.

Refund Application for
Parking Permit
Purpose: Complete this form to receive a refund of your parking permit fee.

Documentation Required: Submit the actual permit with the refund application.
Processing Timeframe: Refund requests must be received within the first two weeks of the semester. Refunds will be processed after the first two weeks of the semester and will take 4 to 6 weeks to receive.
Student Notification: If student paid by credit card, a refund will be applied to the same credit card used. If the student paid by cash or check, the student will receive a check sent to the address of record.
Residency Determination Appeal Form Purpose: Submit this form after you have submitted the Residency Status Change Request and received a denial of in-state residency status.  If you feel that the determination is incorrect, complete this form and submit with documentation that will support your claim for in-state residence status.

Student Program Eligibility Appeal Purpose: Submit this form after you have submitted an Appeal to the Director of Financial Aid, Scholarships and Veterans Services or EOPS and received a denial of benefits. The appeal is heard by the Student Program Eligibility Appeals Committee.
