Student Questionnaire

Please answer all of the following questions.   Some of this information will allow me to get in touch with you when I need to or to advise you better.  Some of it is gathered merely for statistical purposes.  All of it is confidential.  I will not share any of the information you provide here with anyone else without your permission.

4.  What other courses are you taking this semester?  Please list name of course, instructor, number of units, and time class meets.

5.  What is your approximate age:

6.  Approximately how many semester units of college course work have you completed?

7.  What kind of a computer will you use at home for your course work?

IBM-style personal computer (IBM, Dell, Gateway, Compaq, HP, etc.)

9.  What word processing program(s) do you use on your home computer?  (Please indicate version; e.g., MS Word 95, Wordperfect 7.  Do not confuse Microsoft Word with Microsoft Works.  If you don't know what version of the program you have, open it, click Help, on the right of the menu bar at the top of the page, and click About whatever program you are using.  That will give you the version number.) 

10.  What is your Internet service provider?  (For example., AOL, Hotmail, Cox, EarthLink, MSN) If you do not have Internet access, put "none."

11.  Please indicate when you plan to do your work for this course.  This will, of course, be an estimate, and your schedule may change during the semester.   But since you should expect to spend an average of twelve hours each week doing the work for this course, please indicate when you plan to schedule your class work (including class meeting, of course, if you are taking the class on campus).  This will make it easier to schedule group work and to determine when assignments should be due as the semester progresses.

11.  Is there anything else you'd like me to know?  If you have any special circumstances, or expect to have any during the semester, let me know.  Are you planning a business or pleasure trip that will take you away from your class work?  Are you planning elective surgery or other medical treatment?  A wedding?  Give it some thought.

When you have completed the form, click Submit.  If you wish to start over again, click Reset.  Be aware that the Reset button will erase anything you have already written.  If you wish to change just one response, you can select the portion you want to remove and delete just that portion just as you would in your word processor.  After you have submitted the form you should receive a confirmation page.  Save a copy of that page, either on your computer or by printing it out.
